The Assessments tab records standardised clinical outcome measures and scores them consistently, so you can track change over time.
The measures
Qyire ships a broad library of validated instruments, activated per organisation — your clinic sees only the measures it uses. They span several domains:
The complete catalogue — with citations, licences, and validation status — is in the measure reference, which is generated directly from the code so it never drifts.
Scoring and interpretation
Scoring is performed on the server, from your recorded answers — never guessed in the browser. This keeps results reproducible and auditable, which is required for a regulated medical device.
Measures fall into two groups:
The instrument's published severity bands are shown as colour-coded zones. This currently applies to PHQ-9, GAD-7, CORE-10, DASS-21, SCOFF, and WSAS.
Qyire computes and stores the total score, but shows a neutral interpretation. The clinical bands for these measures are still going through sign-off, so Qyire does not present a colour-coded severity until they are approved. You still get the score and its trend; you interpret it with your own clinical judgement.
Worked example — GAD-7
GAD-7 uses validated severity bands across a 0–21 score:

Recording and back-dating
Completed responses are stored against the patient so the Overview and trend views show progress between appointments. When you record a measure that was actually administered earlier, use the "Administered on" date so the timeline reflects when it was really taken, not when it was typed in.
Outcome measures are a clinical decision-support aid. Interpret scores in the context of the whole clinical picture — they do not replace clinical judgement.
